Report
- Use impartial language
- Keep details to minimum, avoid medical jargon
- Should be typewritten, clearly laid out, signed and dated
- Should report functional limitations and outline activities that may or may not be undertaken
- Any health and safety implications should be outlined and the assessment should aim at a positive statement about the patient's abilities
- Offer report to employee prior/same time as employer (GMC Guidance)
